An assessment of your current dental status will be made and your chief reason for seeking treatment will be identified. Any records regarding previous care are welcome.
The extent of your treatment needs guides our initial visit with you. We gather pertinent information to provide accurate diagnosis and appropriate recommendations. We attempt to answer all your questions regarding your dental health and treatment options available.
In the event of complex treatment needs, additional records such as diagnostic models of your teeth and updated radiographic (images) may be made, and further consultation may be necessary. (Additionally, consultation with specialists in other dental disciplines may be necessary.)
Upon calling the office for an Initial Examination appointment, we will be pleased to appoint you as soon as possible, and assist you in all matters regarding scheduling.

We attempt to accommodate our patients with convenient appointment times. Should you need to reschedule your appointment, we appreciate 48 hours notice so that we may provide service to others.
Our new patient forms are available to you in Adobe Acrobat files. For your convenience, we recommend printing and completing these forms at home.
Please remember to bring your completed forms with you for your first appointment. If you have any problems with these files, please call our office and we will mail or fax them to you.
